List of required documents for admitted students

To confirm your registration, please bring following documents to UFAZ main building: 

  • 5 copies of ID Card
  • 6 photos 3x4
  • School attestation (diploma) – original and 2 copies (those students, that did not receive their attestate should get signed and stamped reference from their school of graduation)
  • Biography and copy in Azerbaijani language
  • Medical reference and 2 copies (reference with photo, form 086)
  • For boys: 3 copies of army document
  • Copies of the SEC exam result and graduation result (2 pieces)
