To confirm your registration, please bring following documents to UFAZ main building

• Biography / original and one copy (in Azerbaijani language; handwriting)
• 4 copies of ID Card;
• Medical reference and 2 copies (reference with photo, form 086);
• Diploma and the original or copy of the transcript (those students, that did not receive their diploma should get signed and stamped reference from their universities);
• 6 photos 3x4;
• Job number and password of personal cabinet;
• Copy of the master admission paper (from SEC, personal cabinet);
• Notarial copy of the documents verifying the status of Internally Displaced Person, children of disabled war veterans, children deprived of parental care, children from families of martyrs;
• 3 copies of the army document for boys
